Charles Hackley Middle School

In order for a student to be seen or treated by Hackley Community Health partners located in Charles Hackley Middle School a parent or guardian needs to have a medical consent form completed and on file.

If your child already has an acute or chronic medical condition with a medical support plan in place (such as asthma, diabetes, allergies, seizures, etc.), please also reach out to Hackley Community Care directly. In order for Hackley staff to best assist your child while they are at school, we must have a signed medical release form on file.

Please note: You are not required to change your current medical provider. This consent simply gives Hackley Community Care permission to provide care within the school setting as needed.
